目的 :采用内痔套扎加缚扎固定法外痔剥切缝合术,探索其有效性、安全性,寻找微创的术式。方法 :将纳入标准的72例患者随机分为治疗组和对照组。治疗组采用内痔套扎加缚扎固定法外痔剥切缝合术,对照组采用传统内痔结扎外痔剥切术,观察两组在手术时间、术中出血量、愈合时间、术后疼痛、术后尿潴留、肛周水肿情况作统计分析。结果 :治疗组在术中出血量,愈合时间,术后24、48、72 h疼痛,术后肛周水肿,术后6个月随访疗效等指标均优于对照组(P<0.05),在手术时间、术后尿潴留方面两组无明显差异(P>0.05)。结论 :证明内痔套扎加缚扎固定法外痔剥切缝合术能减小手术创伤、缩短治疗时间、减少术后并发症,其安全有效,值得临床推广。

Objective :To discuss the method of Barron ligation and blocking and fixing operation of mixed hemorrhoids,looking for minimally invasive surgical procedures. Methods :72 patients were randomly divided into treatment and control groups. The treatment group were given Barron ligation and blocking andfixing operation of mixed hemorrhoids and the control group used traditional method. The operative time,blood loss,healing time,postoperative pain,postoperative urinary retention,perianal edema and effectiveness after 6 months were observed for the statistical analysis. Results :The operative time,blood loss,healing time,postoperative pain of 24 hours,48 hours and 72 hours and postoperative perianal edema were better than those of the control group(P<0.05);the operative time and the postoperative urinary retention in both groups had no significant difference(P>0.05). Conclusion :Barron ligation and blocking and fixing operation of mixed hemorrhoids can reduce the surgical trauma,shorten the treatment time and reduce the postoperative complications,and it is safe and effective and worthy of promotion.
