目的:探讨补肾活血法对肾虚血瘀型老年性眩晕患者血流变学的影响。方法:2016年3月—2018年2月选取老年性眩晕并辨证为肾虚血瘀型患者74例,随机分两组,分别37例。对照组采用倍他司汀4 mg,tid,观察组在此基础上采用补肾活血汤(熟地黄、山茱萸、山药、枸杞子、杜仲、桑寄生、川牛膝、泽泻、川芎、赤芍、丹参、炙甘草)治疗,每日1剂,治疗4周。比较两组间临床疗效、眩晕、耳鸣耳聋、失眠、健忘等中医证候积分及血液流变学,记录不良反应状况。结果:对照组有效率78.38%低于观察组有效率94.59%,差异具有统计学意义(P<0.05)。治疗后对照组和观察组眩晕分别为(0.91±0.56)、(0.51±0.32),耳鸣耳聋分别为(0.85±0.42)、(0.41±0.35),失眠分别为(1.02±0.67)、(0.39±0.28),健忘分别为(0.89±0.54)、(0.43±0.36),血浆黏度分别为(1.73±0.17)、(1.56±0.15),全血黏度(高切)分别为(8.63±0.62)、(8.01±0.56),全血黏度(低切)分别为(45.33±4.67)、(42.12±4.26),红细胞聚集指数分别为(6.01±0.07)、(5.92±0.06),对照组和观察组上述指标比较,差异具有统计学意义(P<0.05)。结论:补肾活血法对肾虚血瘀型老年性眩晕患者的疗效确切,可以改善血液流变学和中医证候。

Objective:To investigate the effect of tonifying kidney and activating blood method in the treatment of vertigo in elderly patients with kidney deficiency and blood stasis type in hemorheology.Methods:74 cases of vertigo in elderly patients with kidney deficiency and blood stasis from March 2016 to February 2018 were selected and randomly divided into 2 groups,37 cases each,and the control group received Betahistine 4 mg,tid,and the treatment group received more with Tonifying Kidney and Activating Blood Decoction(Radix Rehmanniae,Cornus officinalis,Dioscorea opposita,Lycium barbarum,Eucommia ulmoides,Taxillus sutcherensis,Achyranthes bidentata,Alisma orientalis,Ligusticum chuanxiong,Radix Paeoniae rubra,Salvia miltiorrhiza and Glycyrrhiza uralensis)daily dose,and 2 groups were treated for 4 weeks. The clinical efficacy,scores of vertigo,tinnitus,deafness,insomnia,forgetfulness TCM syndromes and levels of hemorrheology were compared 2 groups,and the adverse reactions were recorded. Results:The effective rate of the control group 78.38% was lower than the treatment group(94.59%),and the difference was statistically significant(P<0.05). After treatment,the scores of vertigo in the control group and the treatment group were(0.91±0.56)points and(0.51±0.32)points,scores of tinnitus and deafness were(0.85±0.42)points and(0.41±0.35)points,scores of insomnia were(1.02±0.67)points and(0.39±0.28)points,scores of forgetfulness were(0.89±0.54)points and(0.43±0.36)points,and levels of high viscosity of whole blood were(8.63±0.62)points,(8.01±0.56)points,the low viscosity of whole blood were(45.33±4.67)points and(42.12±4.26)points,and levels of erythrocyte aggregation indexes were(6.01±0.07)points and(5.92±0.06)points,all indexes in 2 groups had statistical differences(P<0.05).Conclusion:Tonifying kidney and activating blood method in the treatment of vertigo in elderly patients with kidney deficiency and blood stasis type was effective,and it can improve the hemorheology and TCM syndrome.
