心血管疾病(cerebrovascular disease,CVD)是全球人群死亡的首要病因,近年来其患病率及病死率仍处于持续上升阶段。动脉粥样硬化(atherosclerosis,AS)作为心血管疾病的重要病理基础,通过多种机制导致心肌损伤。在生物-心理-社会医学的模式下,对CVD的治疗不仅局限于西医常规药物和介入治疗等,更需要身心同治,深入开展中医药防治CVD有着重要的临床意义。该研究以“气机—线粒体动力学”为基础阐释“神气舍心”理论,将传统中医概念与现代生物学机制相结合,揭示七情致病、气机逆乱的病理本质之一是线粒体非稳态负荷(allostatic,MAL)造成的细胞损伤,为越鞠丸“调畅气机,御神宁心”治疗“双心疾病”提供科学理论依据,为慢性应激加重AS状态下心肌损伤的中西医结合治疗提供了新的方向和希望。
Abstract:Cerebrovascular disease(CVD)is the leading cause of death in the world,and its prevalence and mortality are still rising in recent years. Atherosclerosis(AS)an important pathological basis of cardiovascular disease,AS causes myocardial injury through multiple mechanisms. In the biopsycho-social medical model,the treatment of CVD is not confined to conventional Western medical drugs and interventional therapies,but more importantly requires simultaneous treatment of the body and mind.In-depth development of traditional Chinese medicine for the prevention and treatment of CVD holds significant clinical significance. This study interprets the theory of “spirit-Qi residing in the heart”based on the “Qi mechanism-mitochondrial dynamics” framework,integrating traditional Chinese medicine concepts with modern biological mechanisms. It reveals that one of the pathological essences of emotional pathogenesis and Qi disorder is cellular damage caused by mitochondrial allostatic load(MAL),providing a scientific theoretical basis for Yueju Pill(越鞠丸)in treating “double heart diseases”by “regulating Qi flow and calming the mind”. This also offers new directions and hope for integrative medicine treatment of myocardial injury in the context of chronic stress-aggravated AS.
